Accountability without confusion across all tasks.

    Switching a template replaces the matrix data live — your manual edits on the current preset will be lost. The Health check updates automatically against whichever preset is active.

    What you are looking at

    Rows are tasks grouped by phase. Columns are roles or people. Click any cell to cycle through R, A, C, I and blank. R is Responsible — does the work. A is Accountable — exactly one per row. C is Consulted — two-way input before decisions. I is Informed — one-way update after decisions. Rows highlighted in red have accountability problems. The Health check tab runs a live audit. The Templates tab loads four industry pre-sets.

    What most PMs miss

    Two As on a single row. It feels collaborative but destroys accountability — when two people are both accountable, each assumes the other is handling it. One A per row is a hard rule with no exceptions. The second miss is C inflation — marking everyone as Consulted to avoid political friction. Every additional C is a communication loop and a potential delay.

    The expert tip

    Run the health check before your kickoff meeting, not after. The errors it surfaces — missing As, duplicate As, unassigned roles — are conversations you need to have while the project is still in setup mode. After kickoff those conversations become political. The health check gives you an objective basis for the discussion: not 'I think we need clarity on ownership' but 'we have six rows with no accountable owner — let us assign them now, in this room.'
